To be eligible for this award, the Officer must meet the following guidelines:

1. Fulfill all mandatory criteria.
2. Fulfill at least 4 of the 6 additional criteria.

To be eligible for consideration, this form and all items must be postmarked and performed by January 30, 1999 and then sent to: Chris Novotny, Lt. Gov. Div. 5/6, 7631 Clarendon Hills Road, Willowbrook, IL 60514

3. Prepare regular written financial reports, and submit an example with this award.	____	____
4. Attend Regional Training Conference during your term in office, or receive sufficient training from your Lt. Governor.	____	____
5. Attend a majority of regular club meetings	____	____
